Applications are now being accepted for 2 Temporary Part Time Clinic Navigators to work on the Mobile.Medical.Support (MMS) Team. The successful applicant’s commitment to this position is 24 hours per week Monday - Friday. The successful applicant must be available to work the following shifts: 8am-4pm, 10am-6pm, or 12pm-8pm and every other weekend. This temporary position will end on or before March 31, 2023. Reporting to the Manager of the Mobile. Medical. Support. (MMS) unit, the Clinic Navigator will assist with the responsibilities of mobile community outreach across Windsor Essex. They will work directly on the mobile MMS unit with the client population and nursing team, directing clients through registration and to the appropriate team member(s) for health care. This is a mobile team that spans throughout Windsor-Essex County as the MMS team is deployed out to High Priority Communities to provide interdisciplinary Primary Care and social supports. ESSENTIAL JOB FUNCTIONS - Interact with MMS management, team members, and community partners to deliver culturally appropriate programs and resources
- Engage with MMS clients to ensure their needs are being heard and met
- Identify translation needs and set up clients with the necessary personnel to deliver adapted services and communications
- Direct patients to the correct team members as it relates to registration, health assessments, vaccines, and/or partner resources
- Pick up and deliver vaccine and supplies to the mobile clinic
- Deliver swab samples from the mobile clinic to the lab
- Display basic customer service and use of iPad/computer
- Assist team members with submitting client referrals, prescriptions, follow-up communications, etc. to ensure all wrap-around client care is being provided
- Support capturing data as needed for program development
- Perform other duties as required
QUALIFICATIONS - Grade 12 education required
- Must be able to obtain a Vulnerable Sector Police Clearance
- Valid G driver’s license required
- Proficient with iPads/Tablets and video applications such as FaceTime, Zoom, etc. required
- Basic proficiency in Microsoft Outlook, Word & Excel required
- Education in a health related field preferred
- Recent experience in a hospital or healthcare environment preferred
- Excellent customer service skills and adherence to the expectations of practice at Erie Shores HealthCare
- Ability to work in a fast paced environment and adapt to rapidly changing priorities
- Demonstrated ability to support and embrace the philosophy of Service Excellence and Patient and Family Centered Care
